Hospital Social Worker

We are looking for an Adult Social Worker to join a Hospital Team.

THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE.

About the team

This team works with patients that have been discharged to the Hospital. This team will supports those within the Hospital with the aim to swiftly move them back into the community setting. Within this team you will be contributing to carrying out Care Act 2014 assessments, MCA's, working under Section 42 and creating care packages. This is a fast paced service with flexibility to work from home on a hybrid basis.

About you

You will be completing the necessary assessments in relation to Adult Social Care. A Social Work Degree/DipSW/CQSW within a minimum of 1 year post ASYE experience is required. Experience of working with 65+, Physical Disabilities, Learning Disabilities, Mental Health/ and or hospital.

• There is an option to be paid £35.00 per hour Umbrella with £150 weekly allowance
