Social Worker (Hospitals)

Join our Frimley Park Hospital Social Work Team as a Social Worker. Work with health and social care colleagues to deliver excellent social work services. As a qualified registered Social Worker, you'll help adults leaving hospital achieve the best outcomes.

Key Responsibilities:

• Conduct comprehensive assessments of individuals' needs, strengths, risks and available support.
• Triage safeguarding referrals and make informed decisions.
• Manage a complex caseload whilst providing supervision for Case Workers and Senior Case Workers and practice advice on complex cases.
• Build positive and professional relationships.

Requirements:

• Social Work qualification and registered with Social Work England (SWE).
• A sound understanding of how to apply a strengths-based approach.
• Understanding of social work issues, legal frameworks and social services' statutory duties.
• Commitment to demonstrating our values and behaviours.
