Learning Disabilities & Autism Service - Building Lives of Independence and Well-Being
Join our dynamic Moving on Service - We believe that people are the experts in their own lives and our approach focuses on understanding what they want to achieve.
We empower individuals by respecting their unique experiences and perspectives, building on their strengths and ensuring they have independence in their lives. Our aim is to provide a safe, compassionate space where people can communicate what support they need to live their lives their way.
As a Senior Practitioner, you will be the professional lead in the team and provide supervision and support to your colleagues, to support their development and skills. You will work with the service management team to ensure that the service develops and relationships with internal and external colleagues are positive and promote a 'one council' approach that embeds positive multi-agency working.
You will manage a small caseload of complex cases, using person-centred approaches for assessments, support planning and reviews. You will focus on promoting wellbeing and independence, in line with the Care Act 2014. We ensure people have advocacy, support and representation during periods of change or challenge, helping people succeed and thrive.
